The Maitland office of Providers for Healthy Living located at 341 N Maitland Ave, Ste 340, Maitland, FL 32751 will be permanently closing on 8/31/24. All patient records will be transferred to the Ohio office of Providers for Healthy Living located at 8351 N High St, Ste 155, Columbus, OH 43235. To request a copy of your records or to have your records transferred to a new provider, please call the office at 407-219-3281 or 614-664-3595.
